Our patient was a 34-year-old woman who was born in Afghanistan and immigrated to Canada as a child in the 1990s. She had no medical history, was not taking any medications, and had no drug allergies. She was a lifelong nonsmoker and did not consume alcohol or any recreational drugs.
The patient presented with shortness of breath and was noted to have crackles in the left lower lobe. She was diagnosed with community-acquired pneumonia and initiated on antibiotic treatment. In the course of her evaluation, electrocardiography demonstrated T-wave inversions, raising suspicion for myocardial ischemia. This prompted further cardiac investigations.
